Most returns trace back to evidence that is missing, unreadable, inconsistent or placed where the reviewer cannot verify it.
1. The photo exists, but proves nothing
A tight serial image without context—or a wide overview with no readable detail—can leave the reviewer unable to connect evidence to location and scope. Pair context with detail.
2. Identifiers do not reconcile
Site IDs, sector names, equipment models and filenames that disagree create avoidable review work. Validate the chain from filename to image to report.
3. A required angle is missing
A near-duplicate is not a substitute for a specific grounding, labeling or completed-work view. Map every image to an explicit requirement.
4. Tests and drawings are stale
Final-looking filenames do not guarantee final content. Check revision, site ID, date and scope before assembly.
5. The package is difficult to review
Evidence can be present and still fail operationally when structure, naming or ordering does not follow customer convention. Build for the reviewer’s path.
